Termination type and connector count

For each end, pick one ferrule type, how many connectors, pin gender and polish. Every connector on an end is the same type, and they must add up to the fiber count. Types that can't do that are crossed out. MPO12 can also run 8 fibers with positions 5–8 left empty, the layout used by transceiver ports.
